RESUMEN

In medical imaging, accurate segmentation is crucial to improving diagnosis, treatment, or both. However, navigating the multitude of available architectures for automatic segmentation can be overwhelming, making it challenging to determine the appropriate type of architecture and tune the most crucial parameters during dataset optimisation. To address this problem, we examined and refined seven distinct architectures for segmenting the liver, as well as liver tumours, with a restricted training collection of 60 3D contrast-enhanced magnetic resonance images (CE-MRI) from the ATLAS dataset. Included in these architectures are convolutional neural networks (CNNs), transformers, and hybrid CNN/transformer architectures. Bayesian search techniques were used for hyperparameter tuning to hasten convergence to the optimal parameter mixes while also minimising the number of trained models. It was unexpected that hybrid models, which typically exhibit superior performance on larger datasets, would exhibit comparable performance to CNNs. The optimisation of parameters contributed to better segmentations, resulting in an average increase of 1.7% and 5.0% in liver and tumour segmentation Dice coefficients, respectively. In conclusion, the findings of this study indicate that hybrid CNN/transformer architectures may serve as a practical substitute for CNNs even in small datasets. This underscores the significance of hyperparameter optimisation.

RESUMEN

Phase modulation is demonstrated in a quantum Stark effect modulator designed to operate in the mid-infrared at wavelength around 10 µm. Both phase and amplitude modulation are simultaneously resolved through the measurement of the heterodyne signal arising from the beating of a quantum cascade laser with a highly stabilized frequency comb. The highest measured phase shift is more than 5 degrees with an associated intensity modulation of 5 %. The experimental results are in full agreement with our model in which the complex susceptibility is precisely described considering the linear voltage dependent Stark shift of the optical resonance.

RESUMEN

Radiomics is a discipline that involves studying medical images through their digital data. Using "artificial intelligence" algorithms, radiomics utilizes quantitative and high-throughput analysis of an image's textural richness to obtain relevant information for clinicians, from diagnosis assistance to therapeutic guidance. Exploitation of these data could allow for a more detailed characterization of each phenotype, for each patient, making radiomics a new biomarker of interest, highly promising in the era of precision medicine. Moreover, radiomics is non-invasive, cost-effective, and easily reproducible in time. In the field of oncology, it performs an analysis of the entire tumor, which is impossible with a single biopsy but is essential for understanding the tumor's heterogeneity and is known to be closely related to prognosis. However, current results are sometimes less accurate than expected and often require the addition of non-radiomics data to create a performing model. To highlight the strengths and weaknesses of this new technology, we take the example of hepatocellular carcinoma and show how radiomics could facilitate its diagnosis in difficult cases, predict certain histological features, and estimate treatment response, whether medical or surgical.

OBJECTIVES: The influenza circulation reportedly declined during the COVID-19 pandemic in many countries. The occurrence of this change has not been studied worldwide nor its potential drivers. METHODS: The change in the proportion of positive influenza samples reported by country and trimester was computed relative to the 2014-2019 period using the FluNet database. Random forests were used to determine predictors of change from demographical, weather, pandemic preparedness, COVID-19 incidence, and pandemic response characteristics. Regression trees were used to classify observations according to these predictors. RESULTS: During the COVID-19 pandemic, the influenza decline relative to prepandemic levels was global but heterogeneous across space and time. It was more than 50% for 311 of 376 trimesters-countries and even more than 99% for 135. COVID-19 incidence and pandemic preparedness were the two most important predictors of the decline. Europe and North America initially showed limited decline despite high COVID-19 restrictions; however, there was a strong decline afterward in most temperate countries, where pandemic preparedness, COVID-19 incidence, and social restrictions were high; the decline was limited in countries where these factors were low. The "zero-COVID" countries experienced the greatest decline. CONCLUSION: Our findings set the stage for interpreting the resurgence of influenza worldwide.

The study's purpose was to assess the safety, feasibility and efficiency of selective arterial embolization (SAE) using N-butyl cyanoacrylate (NBCA) glue before percutaneous cryoablation (PCA) of renal malignancies in patients whose tumor characteristics and/or comorbidities resulted in an unacceptable risk of bleeding. In this single-center retrospective study of 19 consecutive high-risk patients (median age, 74 years) with renal malignancies managed in 2017-2020 by SAE with NBCA followed by PCA, data about patients, tumor and procedures characteristics, complications, renal function and hemoglobin concentration before and after treatment, as well as recurrence were collected. Charlson comorbidity index was ≥4 in 89.5% of patients. Ten patients were treated by antiplatelet and/or anticoagulant therapy. Median tumor largest diameter was 3.75 cm (range, 1-6.5 cm) and R.E.N.A.L. nephrometry score was ≥7 in 80%, indicating substantial tumor complexity. No major complications were recorded and minor complications occurred in 7 patients. No residual tumor was found at 6-week imaging follow-up in 18/19 patients. Tumor recurrence was visible in 1/16 patients at 6-month imaging follow-up. No significant difference was found for renal function after treatment (p = 0.07), whereas significant decrease in hemoglobin concentration was noted (p = 0.00004), although it was relevant for only one patient who required only blood transfusion and no further intervention. SAE prior to PCA is safe and effective for managing renal malignancies in high-risk patients.

Fundamental limits of fiber link are set by non-reciprocal effects that violate the hypothesis of equality between forward and backward path. Non-reciprocal noise arises technically from the set-up asymmetry, and fundamentally by the Sagnac effect when the fiber link encloses a non-zero area. As a pre-requisite for observation of Sagnac effect in fiber links, we present a study on phase noise and frequency stability contributions affecting coherent optical frequency transfer in bi-directional fiber links. Both technical and fundamental limitations of Two-Way optical frequency transfer are discussed. Our model predicts and our experiments substantially verify that the dominant noise mechanism at low Fourier frequencies is the polarization asymmetry induced by the temperature and relative humidity variations impacted on fiber links. The flicker noise floor due to the non-reciprocal noise arising from polarization mode dispersion is evidenced for the first time. We perform a post-processing approach which enables us to remove this polarization noise, improve the long-term stability and remove a frequency bias. We evaluate the uncertainty contributions of all the effects discussed for our 50 km spooled fiber link, dominated by its non-reciprocal noise induced by polarization mode dispersion with uncertainty of 1.9( ± 0.8)( ± 1.2) × 10-20. After correction, the linear drift of the residual phase is as low as 27 yoctosecond/s, leading to an uncertainty of the frequency transfer of 2.6 ( ± 39) × 10-22, confirming its potential for searching for more fundamental effects such as Sagnac effect or transient frequency variation due to dark matter.

INTRODUCTION: The aim of this study was to evaluate the impact of the contouring methods on dose metrics and their predictive value on tumor control and survival, in both situations of pre-treatment and post-treatment dosimetry, for patients with advanced HCC treated with SIRT. METHODS: Forty-eight patients who underwent SIRT between 2012 and 2020 were retrospectively included in this study. Target volumes were delineated using two methods: MRI-based contours manually drawn by a radiologist and then registered on SPECT/CT and PET/CT via deformable registration (Pre-CMRI and Post-CMRI), 99mTc-MAA-SPECT and 90Y-microspheres-PET 10% threshold contouring (Pre-CSPECT and Post-CPET). The mean absorbed dose (Dm) and the minimal absorbed dose delivered to 70% of the tumor volume (D70) were evaluated with both contouring methods; the tumor-to-normal liver uptake ratio (TNR) was evaluated with MRI-based contours only. Tumor response was assessed using the mRECIST criteria on the follow-up MRIs. RESULTS: No significant differences were found for Dm and TNR between pre- and post-treatment. TNR evaluated with radiologic contours (Pre-CMRI and Post-CMRI) were predictive of tumor control at 6 months on pre- and post-treatment dosimetry (OR 5.9 and 7.1, respectively; p = 0.02 and 0.01). All dose metrics determined with both methods were predictive of overall survival (OS) on pre-treatment dosimetry, but only Dm with MRI-based contours was predictive of OS on post-treatment images with a median of 23 months for patients with a supramedian Dm versus 14 months for the others (p = 0.04). CONCLUSION: In advanced HCC treated with SIRT, Dm and TNR determined with radiologic contours were predictive of tumor control and OS. This study shows that a rigorous clinical workflow (radiologic contours + registration on scintigraphic images) is feasible and should be prospectively considered for improving therapeutic strategy.

High-quality frequency transfer based on existing telecommunication fiber links allows state-of-the-art microwave and optical clocks to be compared on a continental and potentially even intercontinental scale. We present a half-a-year-long data set of unidirectional optical frequency transfer over a 2×43km urban fiber link. We observe a relative frequency instability of 8.0×10-16 at 1 s and at best 6.0×10-18 at 106s integration time. Our results show that the unidirectional two-way method gives the possibility to perform comparisons of international primary standards over fiber links faster than over satellite links. Moreover, we investigate the major limiting factors of such a unidirectional setup.

The modification of the ground state properties of light atomic nuclei in the nuclear and stellar medium is addressed, using chemical equilibrium constants evaluated from a new analysis of the intermediate energy heavy-ion (Xe+Sn) collision data measured by the INDRA Collaboration. Three different reactions are considered, mainly differing by the isotopic content of the emission source. The thermodynamic conditions of the data samples are extracted from the measured multiplicities allowing for a parametrization of the in-medium modification, determined with the single hypothesis that the different nuclear species in a given sample correspond to a unique common value for the density of the expanding source. We show that this correction, which was not considered in previous analyses of chemical constants from heavy-ion collisions, is necessary, since the observables of the analyzed systems show strong deviations from the expected results for an ideal gas of free clusters. This dataset is further compared to a relativistic mean-field model, and seen to be reasonably compatible with a universal correction of the attractive σ-meson coupling.

We present a study of the fundamental limit of fiber links using dedicated link architecture. We use an experimental arrangement that enables us to detect the forward and backward propagation noise independently and simultaneously in optical fiber and where the optical phase evolution is expected to be driven by the only contribution of the reference arms of the Michelson interferometer ensemble. In this article, we demonstrate indeed the high correlation between the optical phase evolution and the temperature variation of the interferometer ensemble, leading to a frequency offset of (4.4±2.3)×10-21. Using a simple temperature model and a Bayesian analysis to evaluate the model parameters, we show that the temperature effect can be compensated with post-processing, removing the frequency offset down to (0.5±2.0)×10-21. The residual slope of the optical phase evolution over 33 days is 350 yoctosecond/s. Using a global temperature parameter, we divide these 33 days dataset in four subsets and analyse their uncertainties. We show that they are self-consistent when the temperature is taken into account. This provides an alternative method to evaluate the accuracy of a fiber link, especially when the dataset includes large dead times. The result is finally interpreted as a test of the reciprocity of the propagation delay in an optical fiber. This unprecedented transfer capability could enable the comparisons of future optical clocks with expected performance at 10-20 level and open new possibilities for stringent tests of special and general relativity.

The transport of supercritical fluids is a determining factor for several geological processes and fundamental in predicting natural resource accumulation and distribution. Calcite, ubiquitous in most geological environments, may contain supercritical CO2 trapped under the form of fluid inclusions that may move through grain boundaries affecting the rock physical properties. However, despite macroscopic evidence for this process, until recent it was not possible to characterize this process at the nano-scale due to the difficulty of such observations. In this study, we report nanometer-scale observations on calcite crystal surfaces and demonstrate that stress with absence of visible deformation produces fluid leakage from fluid inclusions. Atomic Force Microscopy scanning experiments on freshly cleaved calcite crystals containing visible fluid inclusions revealed the spontaneous formation of nanometer-scale hillocks on flat crystal terraces in only a few minutes, without evidence of surface dissolution. The fact the hillocks formed on flat surface in a short time was unexpected and suggests deposition of material from the inner crystal to the surface through small-scale fluid migration. We estimated the rate of this fluid mobility is by several orders of magnitude higher than the diffusion rate through vacancies estimated in calcite crystals showing that CO2-rich fluids through micro-pore and nano-pore spaces is in reality much higher than previously assumed using current predictive models.

We present a hybrid fiber link combining effective optical frequency transfer and evaluation of performances with a self-synchronized two-way comparison. It enables us to detect the round-trip fiber noise and each of the forward and backward one-way fiber noises simultaneously. The various signals acquired with this setup allow us to study quantitatively several properties of optical fiber links. We check the reciprocity of the accumulated noise forth and back over a bi-directional fiber to the level of 3.1(±3.9) × 10-20 based on a 160000s continuous data. We also analyze the noise correlation between two adjacent fibers and show the first experimental evidence of interferometric noise at very low Fourier frequency. We estimate redundantly and consistently the stability and accuracy of the transferred optical frequency over 43 km at 4 × 10-21 level after 16 days of integration and demonstrate that a frequency comparison with instability as low as 8 × 10-18 would be achievable with uni-directional fibers in urban area.

We report a cascaded optical link of 1100 km for ultra-stable frequency distribution over an Internet fiber network. The link is composed of four spans for which the propagation noise is actively compensated. The robustness and the performance of the link are ensured by five fully automated optoelectronic stations, two of them at the link ends, and three deployed on the field and connecting the spans. This device coherently regenerates the optical signal with the heterodyne optical phase locking of a low-noise laser diode. Optical detection of the beat-note signals for the laser lock and the link noise compensation are obtained with stable and low-noise fibered optical interferometer. We show 3.5 days of continuous operation of the noise-compensated 4-span cascaded link leading to fractional frequency instability of 4x10(-16) at 1-s measurement time and 1x10(-19) at 2000 s. This cascaded link was extended to 1480-km with the same performance. This work is a significant step towards a sustainable wide area ultra-stable optical frequency distribution and comparison network at a very high level of performance.

We describe an ultrasonic instrument for continuous real-time analysis of the fractional mixture of a binary gas system. The instrument is particularly well suited to measurement of leaks of a high molecular weight gas into a system that is nominally composed of a single gas. Sensitivity < 5 × 10(-5) is demonstrated to leaks of octaflouropropane (C3F8) coolant into nitrogen during a long duration (18 month) continuous study. The sensitivity of the described measurement system is shown to depend on the difference in molecular masses of the two gases in the mixture. The impact of temperature and pressure variances on the accuracy of the measurement is analysed. Practical considerations for the implementation and deployment of long term, in situ ultrasonic leak detection systems are also described. Although development of the described systems was motivated by the requirements of an evaporative fluorocarbon cooling system, the instrument is applicable to the detection of leaks of many other gases and to processes requiring continuous knowledge of particular binary gas mixture fractions.

In this article, we consider a new regression model for counting processes under a proportional hazards assumption. This model is motivated by the need of understanding the evolution of the booking process of a railway company. The main novelty of the approach consists in assuming that the baseline hazard function is piecewise constant, with unknown times of jump (these times of jump are estimated from the data as model parameters). Hence, the parameters of the model can be separated into two different types: parameters that measure the influence of the covariates, and parameters from a multiple change-point model for the baseline. Cox's semiparametric regression can be seen as a limit case of our model. We develop an iterative procedure to estimate the different parameters, and a test procedure that allows to perform change-point detection in the baseline. Our technique is supported by simulation studies and a real data analysis, which show that our model can be a reasonable alternative to Cox's regression model, particularly in the presence of tied event times.

We report an optical link of 540 km for ultrastable frequency distribution over the Internet fiber network. The stable frequency optical signal is processed enabling uninterrupted propagation on both directions. The robustness and the performance of the link are enhanced by a cost effective fully automated optoelectronic station. This device is able to coherently regenerate the return optical signal with a heterodyne optical phase locking of a low noise laser diode. Moreover the incoming signal polarization variation are tracked and processed in order to maintain beat note amplitudes within the operation range. Stable fibered optical interferometer enables optical detection of the link round trip phase signal. The phase-noise compensated link shows a fractional frequency instability in 10 Hz bandwidth of 5 × 10(-15) at one second measurement time and 2 × 10(-19) at 30,000 s. This work is a significant step towards a sustainable wide area ultrastable optical frequency distribution and comparison network.

We demonstrate a cascaded optical link for ultrastable frequency dissemination comprised of two compensated links of 150 km and a repeater station. Each link includes 114 km of Internet fiber simultaneously carrying data traffic through a dense wavelength division multiplexing technology, and passes through two routing centers of the telecommunication network. The optical reference signal is inserted in and extracted from the communication network using bidirectional optical add-drop multiplexers. The repeater station operates autonomously ensuring noise compensation on the two links and the ultra-stable signal optical regeneration. The compensated link shows a fractional frequency instability of 3 x 10(-15) at one second measurement time and 5 x 10(-20) at 20 hours. This work paves the way to a wide dissemination of ultra-stable optical clock signals between distant laboratories via the Internet network.

We transferred the frequency of an ultrastable laser over a 108-km-long urban fiber link comprising 22 km of an optical communications network fiber simultaneously carrying Internet data traffic. The metrological signal and the digital data signal were transferred over two different frequency channels in a dense wavelength-division multiplexing scheme. The metrological signal was inserted in and extracted from the communication network using bidirectional off-the-shelf optical add-drop multiplexers. The link-induced phase noise was measured and canceled with a round-trip technique using an all-fiber-based interferometer. The compensated link showed an Allan deviation of a few 10(-16) at 1 s and below 10(-19) at 10,000 s. This work paves the way to a wide dissemination of ultrastable optical clock signals between distant laboratories via the Internet.

We demonstrate a new, simple technique for measuring IR frequencies near 30 THz by using a femtosecond (fs) laser optical comb and sum-frequency generation. The optical frequency is directly compared with the distance between two modes of the fs laser, and the resultant beat note is used to control this distance, which depends only on the repetition rate of the fs laser. The absolute frequency of a CO2 laser stabilized onto a SF6 two-photon line has been measured for the first time to the authors' knowledge. This line is an attractive alternative to the usual saturated absorption OsO4 resonances used for the stabilization of CO2 lasers. First results demonstrate a fractional Allan deviation of 3 x 10(-14) at 1 s.
